this is from my little commuter car, It seems after i replaced the stub axle i neglected to double check the torque on the axle nut. this is what happened. Remember even if you are tired and its late and you just need to get it done so you can go to work in the morning, give those nuts and bolts a quick double check. here is a link loose axle nut
